एमबिबिएस शुल्क सिफारिस,सरकारीमा ३२, निजीमा ६२ लाखसम्म

चिकित्सा शिक्षाअन्तर्गत एमबिबिएस तह अध्ययनका लागि गठित समितिले नयाँ शुल्क सिफारिस गरिएको छ। शिक्षा मन्त्रालयद्वारा गत असारमा गठित शुल्क निर्धारण समितिले विभिन्न वर्गीकरणका आधारमा प्रतिविद्यार्थी ४८ देखि ६२ लाख रुपैयाँसम्म शुल्क लिन सिफारिस गरेको हो। यो खबर हामीले आजको नागरिक दैनिकबाट लिएका हौ।

न्यूनतम शुल्क ३२ लाख रुपैयाँ भने सरकारी प्रतिष्ठान एवं संस्थामा मात्र लागू हुनेछ । अधिकांश कलेजका सबैजसो सिट ४८ लाख रुपैयाँ शुल्क लिन पाउने गरी सिफारिस गरिएको समितिका एक सदस्यले जानकारी दिए ।

प्रधानमन्त्री पुष्पकमल दाहालसमक्ष समितिले शनिबारै प्रतिवेदन बुझाएको हो। दाहालले तत्कालै मन्त्रिपरिषद् लगेर प्रतिवेदन लागू गराउने वचन दिइसकेका छन् ।  समिति संयोजक डा. शिवकुमार राईले भने प्रधानमन्त्रीलाई प्रतिवेदन बुझाइएकाले मन्त्रिपरिषद्ले पास नगरेसम्म केही बताउन नमिन्ने बताए ।

गत असार २६ गते मन्त्रिपरिषद् बैठकले डा. शिवकुमार राईको संयोजकत्वमा सातसदस्यीय चिकित्सा शिक्षा शुल्क निर्धारण समिति गठन गरेको थियो । समितिले अधिकतम सिट संख्या १ सय १५ हुने कलेजका लागि ४८ लाख रुपैयाँ निर्धारण गरेको हो ।

कलेजको नियमित लागत, छात्रवृत्ति लागतसमेत हेरेर शुल्क निर्धारण गरिएको समितिको दाबी छ । स्रोतका अनुसार समितिले छात्रवृत्ति, रेगुलर र कमर्सियल गरी तीन प्रकारका सिट तोकेको छ जसअनुसार कम्तीमा कुल सिटको १÷३ मात्रै कर्मसियल हुने र ती सिटमा ४८ लाख शुल्कमा २५ प्रतिशतसम्म थप गरेर ६२ लाखसम्म लिन सकिने समितिले सुझाव दिएको छ । कर्मसियल सिटका लागि छुट्टै परीक्षा लिनसमेत समितिको सिफारिस छ।

यसअघि त्रिवि पूर्वउपकुलपति केदारभक्त माथेमा प्रतिवेदनले उपत्यकाबाहिर र भित्र दुई आधारमा शुल्क तोक्न भनेकोमा यो आधार वैज्ञानिक नभएको समितिको निष्कर्ष छ । चिकित्सा शिक्षासम्बन्धी माथेमा कार्यदलले उपत्यकामा ३५ लाख र उपत्यकाबाहिर ३८ लाख शुल्क सिफारिस गरेको थियो । सो शुल्कसीमा एक वर्षमा र त्यसपछि प्रत्येक दुई वर्षमा आर्थिक वृद्धिदरका आधारमा बढाउँदै लैजाने सिफारिस कार्यदलको थियो तर समितिले यो सर्त र सुझावभन्दा बाहिर गएर सिफारिस गरेको देखिएको छ ।

समितिमा यसपटक निजी कलेज सञ्चालक नै सदस्य भएर आएका थिए । सरकारले समिति बनाएपछि केही चिकित्सकले सुरुमै बस्न इन्कार गरेपछि समिति परिमार्जन गरिएको थियो । अघिल्लो वर्ष शुल्क सुझाव अटेर गरेका निजी क्षेत्र पनि समितिमा सम्मिलित भएकाले यसपटकको लागू हुने सरकारी बुझाइ छ ।

समितिमा डा. श्रीकृष्ण गिरी, डा. रोहितकुमार पोखरेल, डा. राजेश कोजू, डा. भोला रिजाल, डा. तुलसी थपलिया र महेश गुरागाईं सदस्य छन् ।

माधवकुमार नेपालको प्रधानमन्त्रीत्व कालमा २०६६ सालमै सरकारले चिकित्सा शिक्षा शुल्क तोक्ने प्रयास गरे पनि निजी कलेज सञ्चालकको अटेरीले लागू हुन सकेको छैन । त्यतिबेला योजना आयोगका तत्कालीन सदस्य तीर्थ खनियाँको संयोजकत्वमा समिति बनाइएकोमा पछि सरकार परिवर्तनसँगै डा. राईले नै योजना आयोगका तर्फबाट समितिको नेतृत्व गरेर प्रतिवेदन बुझाएका थिए ।

त्यतिबेला राई समितिले ३२ लाख रुपैयाँ शुल्क तोक्न सिफारिस गरे पनि निजी कलेजका सञ्चालकको दबाबमा लागू हुनै सकेन । सरकारले गत वर्ष माथेमा प्रतिवेदनका आधारमा शुल्कसीमा लागू गर्ने निर्णय गरे पनि कलेजहरुको अटेरीकै मनपरी जारी छ ।

स्वस्थ्य बनाउँ निराेगी भएर बाँचाै

सेप्टेम्बर २९ लाई विश्व मुटु दिवसका रूपमा संसारभर मनाइन्छ । यसको मुख्य उद्देश्य स्वस्थकर मुटु राख्न वातावरण बनाउन सबैलाई उत्प्रेरित गराउनु हो ।

मुटु र रक्तसञ्चारसम्बन्धी सबै रोग जस्तै- कोरोनरी मुटुरोग (एन्जाइना र हृदयाघात) र स्ट्रोक मान्छे मर्ने रोगहरूमध्ये प्रमुख हो । अध्ययनअनुसार दक्षिण एसिया र अफ्रो क्यारिबियनका मान्छेलाई कोरोनरी मुटुरोग लाग्ने सम्भावना अन्यत्रकालाई भन्दा बढी हुन्छ । समयमै सजगता अपनाएर जीवनशैली परिवर्तन गरे मुटुरोगबाट बच्न या सावधान हुन महत्वपूर्ण भूमिका खेल्नसक्छ ।

किन बढ्छ मुटुरोग

धूमपानः तपाईं धूमपान गर्नुहुन्छ भने स्वास्थ्य सुधारका निम्ति यसलाई छोड्नु नै सबभन्दा महत्वपूर्ण निर्णय हुनेछ । चुरोटमा निकोटिन र कार्बन मोनोअक्साइडजस्ता हजारौं केमिकल हुन्छन् । यसले मुटुको धड्कन बढाएर मुटुलाई बढी काम गर्न बल गर्छ । रक्तचाप बढाउँछ र रगतलाई बाक्लो तथा टाँसिने बनाउँछ । यसले रगत जम्न सजिलो पार्छ ।

कार्बन मोनोअक्साइडजस्ता केमिकलले मुटु र शरीरमा चाहिने अक्सिजन कम गराउँछ । यसले कोलेस्ट्रोललाई जान दिन्छ, जुन जम्मा भई रक्त कोशिकाको नलीलाई साँघुरो बनाउँछ ।
धूमपानले मुटुरोगका अलावा स्ट्रोक, फोक्सोको क्यान्सर, आस्मा, दम, अल्सर आदि रोग जन्माउन सक्छ ।

पारिवारिक इतिहास

कोरोनरी मुटुरोग वंशानुगत रूपले परिवारमा सर्नसक्छ । रगतको नाताजस्तै बाबु, आमा, फुपू, मामा आदिलाई यस्तो रोग लागेको छ भने जोखिमका तत्वहरूलाई निवारण गर्न के गरिएको छ, यो पनि महत्वपूर्ण कदम हो ।

उच्च रक्तचाप

चिन्तामा रहेर व्यायाम गर्दा रक्तचाप बढ्छ । यो सामान्य हो । तर, केही नगरीकन आरामले बस्दा पनि रक्तचाप बढ्छ भने यसलाई असाधारण मान्न सकिन्छ । उच्च रक्तचाप धूमपानबाट पनि हुनसक्छ । यसले मुटु र रक्तकोषिकालाई बढी बल पार्छ, जसका कारण रगत बाक्लो, कडा र कम लचकदार बनाउँछ ।
तौल बढी छ भने मुटुरोग लाग्ने सम्भावना बढी हुन्छ । बढी तौल भएपछि उच्च रक्तचाप, मधुमेह, रगतमा बढी कोलेस्ट्रोल हुने जोखिम हुन्छ ।
उच्च रक्तचाप छ भने डाक्टरको सल्लाहबमोजिम औषधि खानुपर्छ । बढी वजन हुनबाट टाढा रहने, धूमपान त्याग्ने, नुन खान कम गर्ने, मदिरा खान कम गर्ने, चिन्ता घटाउने, केही व्यायाम गर्ने ।

  1. बढी तौल — तौल बढी छ भने मुटुरोग लाग्ने सम्भावना बढी हुन्छ । बढी तौल भएपछि उच्च रक्तचाप, मधुमेह, रगतमा बढी कोलेस्ट्रोल हुने जोखिम हुन्छ ।
  2. चिन्ता — चिन्ताको प्रकृति र अर्थ हरेक मानिसमा फरक हुनसक्छ । एउटा व्यक्तिलाई भएको चिन्ता अर्को व्यक्तिलाई नहुन सक्छ । केही निश्चित मात्रामा चिन्ता हुनु आवश्यक हुन्छ, यसले हाम्रो जीवनमा काम गर्न हौसला र मद्दत पु‍र्‍याउँछ । त्यति बेला समस्या हुन्छ, जब चिन्ता हाम्रो वशबाट बाहिर जान्छ । बढी चिन्ताले धूमपान, मद्यपान, आलस्यजस्ता कुलतलाई प्रोत्साहन गर्छ । आराम गर्ने तरिका सिकेर चिन्ताको समस्या कम गर्न सकिन्छ ।
  3. व्यायामको कमी — मुटु एउटा मांसपेशी भएको हुनाले यसलाई तन्दुरुस्त राख्न हरेक दिन व्यायामको आवश्यकता हुन्छ । योजनाबद्ध व्यायाम जस्तै पौडिने, दौडने, हिँड्ने गर्नुपर्छ । जीउ तताउने गर्नुपर्छ । यो अति महत्वपूर्ण हो कि विस्तारै मांसपेशी र जोर्नीलाई खुकुलो गर्नुपर्छ । यसले मुटुको मांसपेशीमा रगत पुर्‍याएर सुधार गर्छ ।
    व्यायाम गर्दा सजिलोसँग बोल्न सक्नुपर्छ । व्यायाम सकिनुअगाडि जीउ चिस्याउने गर्नुपर्छ । यो महत्वपूर्ण हो कि विस्तारै मुटुको धड्कन कम गर्नुपर्छ । शरीरलाई साधारण अवस्थामा राख्ने कोसिस गर्नुपर्छ । शरीरअनुरूप तन्दुरुस्ती राख्नुपर्छ । नियमित हिँड्नु नै उत्तम व्यायाम हो 
  4. रगतमा चिल्लो — कोलेस्ट्रोलः कोलेस्ट्रोल भनेको चिल्लो पदार्थ हो, जुन शरीरले प्राकृतिक रूपबाट नै बनाउँछ । यो कलेजोले स्याचुरेटेट फ्याटबाट बनाउँछ । केही चिल्लोहरू मुटुलाई राम्रो (एचडीएल) र कोही नराम्रो (एलडीएल) हुन्छन् । खानामा धेरै चिनी या मदिरा भएमा यसको लेभल बढ्छ । मधुमेह भएको व्यक्तिमा पनि यसको लेभल बढी हुन्छ ।
स्वस्थकर मुटुका लागि खाना

स्ट्राची खानाः हामीले खाने खानामा ३३५ स्ट्राची हुनुपर्छ । यस प्रकारको खानामा शक्ति र पोषणयुक्त तत्व पाइन्छ । यसमा फाइबर, क्याल्सियम, आइरन, भिटामिन बीहरू पाइन्छ । हरेक पटकको खानामा स्ट्राची खाना हुनुपर्छ । यसलाई पकाउँदा या खाँदा धेरै क्यालोरी बढाउने जस्तै तेल, घिउ, चिनी थप्नु हुँदैन । होलग्रेन खानालाई पचाउन धेरै समय लाग्छ । त्यसैले धेरै बेरसम्म भोक लाग्दैन । यी खानाका उदाहरणमा होलग्रेन पाउरोटी, पास्टा, नफलेको चामल आदि पर्छन् ।

फलफूल र सब्जीः घटीमा पनि दिनको पाँच पोर्सन विभिन्न किसिमका फलफूल र सब्जीहरू खानुपर्छ । एक पोर्सन भनेको करिब ८० ग्राम हुन्छ (ताजा फलफूल, सब्जी)

माछाः माछा बढी खाऔं । हप्तामा घटीमा एक पोर्सन (१४० ग्राम) तेलयुक्त माछा खानु उत्तम हुन्छ । माछामा प्रोटिन, अरू धेरै भिटामिन र मिनरलहरू पाइन्छन् ।

स्याचुरेटेट फ्याट र चिनी कम गरौः जनावरबाट पाइने चिल्लो जस्तै बोसो, घ्यू, बटर र चिनी, सकेसम्म कम खानुपर्छ । यसको सट्टा कम परिणाम अनस्याचुरेटेट फ्याट जस्तै सूर्यमुखी तेल, तोरीको तेल, ओलिभको तेल, तेलयुक्त माछाहरू, नट्स अनि बियाँ खानुपर्छ । धेरै मिठाई, चिनीयुक्त पेय पदार्थ धेरै कम गर्नुपर्छ ।
नुन कम खाऔः दिनमा ६ ग्रामभन्दा कम नुन खानुपर्छ । धेरै खाएपछि स्वास्थ्यमा हुने जोखिमहरू जस्तै उच्च रक्तचाप धेरै बढ्छ । प्रौढ मान्छेले दिनमा ६ ग्रामभन्दा कम नुन खानुपर्छ ।

प्रशस्त पानीः हाम्रो शरीरलाई पानी प्रशस्त मात्रामा चाहिन्छ । ठाउँको मौसमअनुरूप पानी पिउनुपर्छ ।

मधुमेह

जब शरीरले रगतमा भएको चिनीलाई तोड्न सक्दैन तब मधुमेह हुन्छ । रगतमा धेरै चिनी भएपछि यो बाक्लो र टाँसिने हुन पुग्छ । यसको मतलब मुटुले शरीरभरि रगत पम्प गरी पठाउन कडा काम गर्नुपर्छ । मधुमेह भएको मान्छेले सधैं यसलाई नियन्त्रण गर्नुपर्छ । पिसाब र रगत समय समयमा जाँच्नुपर्छ । मधुमेहको नियन्त्रणका लागि खाना महत्वपूर्ण उपचारविधि हो । खाना सधंै नियमित छाक नछोडीकन, धेरै बेरसम्म भोको नबसेर, चिनीयुक्त खाना कम गर्ने, चोकरयुक्त खानाका साथै प्रशस्त फाइबर आदि खाने गर्नुपर्छ ।

प्रभाव पार्ने अरू तत्व

धेरै पुरुषलाई मुटुको समस्या आधाआधी उमेरमा पुगेपछि सुरु हुन्छ । महिलालाई अलि उमेर बढी भएपछि देखिन्छ । मुख्यतः यसका दुई कारण छन् ।
  1. धेरै जोखिमका तत्वहरू महिलाभन्दा पुरुषमा बढी पाइन्छ । जस्तै— धूमपान, उच्च रक्तचाप, बढी वजन आदि । महिलाले शारीरिक हिसाबले पनि धेरै काम गरी जीवनशैली स्वस्थकर बनाएका हुन सक्छन् ।
  2. कोलेस्ट्रोलको लेभल पुरुष र महिलामा फरक पाउन सकिन्छ । महिलालाई प्राकृतिक रूपबाट नै हार्मोनले सुरक्षा प्रदान गर्छ । यसको परिणाम कलिलो उमेरमा कम संख्यामा महिला मुटुका रोगी बन्छन् । तर, महिनावारी हुन छोडेपछि पुरुषसरह नै हुन्छन् ।
मदिरा

थोरै परिणामको मदिराले मुटुलाई राम्रो गर्छ भन्ने पुरानो मान्यतालाई हालै गरिएको अध्ययनले अस्वीकार गरेको छ । अध्ययनलाई मनन गरी बेलायती सरकारले पिउनैपर्ने मानिसलाई हप्तामा १४ युनिटभन्दा कम भन्ने सुझाव दिएर सावधानी अपनाएको छ । एक युनिट भनेको एक पेग (२५ एमएल) ह्विस्की, आधी प्वाइन्ट बियर, सानो गिलास वाइन, १२५ मिलिलिटर हो ।

धेरै मदिराले मुटुको मांसपेशीलाई नोक्सान पु‍र्‍याउँछ । मिर्गौला र अरू रोग लाग्ने जोखिम पनि रहन्छ । मदिरामा धेरै क्यालोरी हुने हुँदा मोटो मान्छेले विशेष विचार गर्नुपर्छ । वर्षमा घटीमा एकपल्ट रगत जाँच्ने, आफ्नो रक्तचाप, रगतमा चिनीको मात्रा र कोलेस्ट्रोल थाहा पाउने, त्यही अनुरूप जीवनशैली परिवर्तन गरेर मुटुरोगबाट टाढा रहनु भन्ने सन्देश नै मुटु दिवसको हो ।
